How does a donor use a requisition?

A requisition identifies the requested testing service. Unity’s available automated system creates the form for the donor to take to a network collection site.

  1. Confirm the request. Identify the required service and the donor’s location with your program contact.
  2. Arrange the requisition. Use the agreed program process to obtain the donor’s requisition form.
  3. Confirm the collection location. Check that the selected site offers the requested service and confirm its hours or appointment requirements before traveling.
  4. Take the form to the site. Follow the collection instructions supplied for the testing event. Contact your program representative if the request or location needs clarification.
