FAQ
Share your driver’s location and the service needed. Unity helps identify a convenient network location offering DOT physicals with an FMCSA National Registry medical examiner, subject to provider availability.
We track expiration dates proactively and send reminders well ahead of the deadline, so no driver is caught off guard.
Which federal rules govern DOT physicals?
49 CFR 391.41 sets physical qualification standards, 391.43 covers the medical examination and certification, and 391.45 addresses when examinations are required. CDL and commercial learner’s permit self-certification requirements appear in 49 CFR 383.71. Applicability depends on the driver’s operations and any valid exceptions. 49 CFR 391.41
Who can perform a DOT physical for an interstate commercial driver?
The exam must be performed by a medical examiner listed on FMCSA’s National Registry. Unity coordinates access to providers; the examiner assesses the driver and makes the medical qualification decision. FMCSA National Registry
How often does a driver need a new medical examination?
The general federal interval is no more than 24 months, but a shorter certification period or specific condition may require earlier re-examination. An injury or illness that impairs normal driving duties can also trigger a new examination. Follow the examiner’s determination, the certificate expiration date, and applicable requirements. 49 CFR 391.45
What is CDL medical self-certification?
Drivers tell their licensing state whether they operate in interstate or intrastate commerce and in an excepted or non-excepted category. That classification affects medical certification obligations. It is separate from the medical exam, and drivers must keep the classification accurate as their operations change. 49 CFR 383.71
Are state CDL medical requirements the same everywhere?
Federal requirements apply to covered interstate operations, while intrastate medical standards, exceptions, and licensing procedures can vary by state. Check the agency that issued the CDL for its current requirements and medical-status process. An exam appointment alone does not confirm that the state’s license record is current. FMCSA self-certification guidance
What should Florida CDL holders know?
Florida Statutes section 322.59 ties CDL issuance and continued commercial driving eligibility to required medical certification under 49 CFR 383.71. Failure to comply can lead to disqualification. Florida drivers should confirm their self-certification category and current medical status with FLHSMV, including the current process for updating the licensing record. Florida Statutes § 322.59
Is a DOT physical the same as a DOT drug test?
No. The physical evaluates medical fitness under the applicable driver qualification rules. DOT drug and alcohol testing follows a separate process under 49 CFR Part 40 and the relevant DOT agency rules. Tell Unity which services are needed so the location and appointment can be coordinated appropriately. 49 CFR 391.43
General information reviewed September 28, 2026. Requirements depend on the driver’s operations, medical circumstances, and licensing state. Confirm individual requirements with the medical examiner and state driver licensing agency.
